Group Home Facility Manager is the primary liaison between the Direct Support Professionals, families/guardians, facility
staff, and the County Case Management Team. Ensure all programmatic features of the client’s Individual Service Plan
and Individual Program Plan are carried out in an effective manner. Manage staff and responsible for the decision making
necessary for the operation of their department. Key hands on direct care staff role.
Coordinating services and care for all residents in the home
Providing a secure and responsive environment so that each resident may have the opportunity to maximize his/her
physical, emotional, intellectual and spiritual capabilities
Maintaining the day-to-day operation of the home, including the overall supervision, scheduling and coordination of
staff in the home
Demonstrates a working knowledge of and complies with company Policies and Procedures
Participates in development of resident Initial Service Plan and completes quarterly Service Plan evaluation and
updates for submission to placement agencies
Completes monthly assessment of each resident within home for submission to placement agencies
Supports residents in all aspects of residential services, including educational, community services, employment,
placement agency and medical needs
Provides case management with counties and families
Maintains professional boundaries with participants, families, employees and supervisor
Works closely with Executive Director, Client Support Manager and Intake Coordinator to maintain case files for
Responsible for implementation of Life Skills Learning for residents
A minimum of an Associates’ degree is required in related field; a Bachelor’s degree is preferred or equivalent work
At least 3 years of FT direct care service with persons with intellectual disabilities, human services, healthcare or
special education is needed.
Demonstrated exceptional leadership ability
Strong computer and writing skills are required
Must demonstrate the emotional maturity and patience necessary to understand and accept the social, psychological
and emotional needs of adults with developmental disabilities, nurturing their self-sufficiency and enhancing their
quality of life
Must have the ability to represent KYS in a positive manner within the community.
Must be at least 21 years of age, have a valid driver’s license and acceptable driving record
