The purpose of Aftercare Support is to provide guidance and follow-up for clients transitioning from our Youth Independent Living programs. The aftercare follow-up plan begins 30-days after emancipation from the foster care system and includes check-ins for assistance & support. The Aftercare plan will consist of the following:
